A large, academic health system in Northern Indiana is seeking an Ambulatory Care Clinical Pharmacy Specialist to join a rapidly expanding ambulatory pharmacy program. These embedded clinic roles provide direct patient care through collaborative drug therapy management and focus on a combination of anticoagulation and diabetes management, with additional opportunities in specialty clinics. This is not a diabetes-only position. Pharmacists work closely with physicians, advanced practice providers, and care teams to optimize chronic disease management and support the continued growth of ambulatory pharmacy services.
Candidates must have completed a PGY1 Ambulatory Care residency, with PGY2 Ambulatory Care or equivalent experience preferred. Current residents will be considered. Pharmacists with PGY1-only training may initially focus more heavily on anticoagulation, with expansion into diabetes management over time. Board certification (BCACP) is preferred. Strong communication skills, clinical confidence, and the ability to build long-term relationships with patients and providers are essential for success in this role.
